Application for Admission
Parents and legal guardians are requested to complete the Application for Admission form and attach all required documents for submission.
The form includes learner details, educational and medical information, parent or guardian details, declarations, conditions for admission, and financial terms and conditions.

- Please fill in black ink.
- All areas must be filled in legibly.
- All required documents must be attached for submission.

- Copy of learner's birth certificate
- Certified ID copies of both parents or legal guardians
- Copy of learner's vaccination records
- Proof of registration fee payment
- Fee clearance from previous school
- Disciplinary record from previous school
- Completed afternoon care application, where applicable
Additional Requirements
- Clinic card for Grade 1
- Transfer letter and report from previous school for Grades 2–7
- Proof of employment for the account holder
- Fee clearance certificate from previous school
